What you will be doing

Lead the functionality and implementation of shared care plans across the Canterbury health system and regionally across the South Island: This includes:

  • Managing the functional, technological and integration requirements for the wider implementation of care planning solutions across the Waitaha | Canterbury and the South Island health systems and partner organisations.
  • Lead the design and refinement of electronic care plan functionality working alongside clinical leaders.
  • Working with key stakeholders to facilitate implementation and use of shared care planning across Waitaha | Canterbury.
  • Working with Te Waipounamu | South Island champions to facilitate the wider implementation of care planning.
  • Leading the small Christchurch Based Shared Care planning programme team.

This is a part time role 20-24 hours per week, flexibility of days and hours to be discussed at interview level.

Location of work is Pegasus Health, 401 Madras Street, Christchurch.

What you will bring

  • Ability to lead and drive positive changes.
  • Data analysis and literacy.
  • Knowledge of both primary, community, and secondary health sectors.
  • An understanding of formal project management concepts and methods.
  • Knowledge of the New Zealand health digital landscape and electronic solutions used across services.
  • A high level of stakeholder management and facilitation skills.
  • At least 4-5 years’ experience in the health sector.
