The Acute Demand programme is a Canterbury wide network of services designed to provide community-based care for patients either at risk of presentation to the Emergency Department, admission to hospital, or able to discharge home with advanced nurse assessments. 

The service is designed to:

  • Support and assist general practice teams to help patients who are acutely unwell to remain in thcommunity for their treatment.
  • Help to facilitate early discharge from hospital for mild-moderately unwell patients where it is deemed safe to manage their care in the community.
  • The service is aimed towards patients who have an acute episode of illness that may otherwise be admitted to hospital.

Ko tāuake mahi | What you will be doing

As an Acute Demand Registered Nurse, you will work in a variety of roles within the Acute Demand Nursing Team delivering  community-based nursing caretreating patients in our clinic based at the 24-Hour Surgery, working as the Acute Demand Coordinator and helping to facilitate supported discharge working in the Hospital Link role. We have two positions available for up to 32 hours per week each. 

Your working day will vary as you:

  • Carry out comprehensive nursing assessments and care plans.
  • Deliver a range of interventions for patients with an acute episode of illness that would typically respond to treatment within 3-5 days. Such conditions may include cellulitis, pyelonephritis, urinary retention, sub/supra therapeutic INR management, respiratory illnesses, and Heart Failure.
  • Work collaboratively with the following teams: general practice, hospital, ambulance, and community services around short term care planning and available treatment options.
  • Work within the coordination team, as a nurse coordinator, receiving referrals from general practice, the 24-HourSurgery and the hospital. Providing clinical support for Acute Demand Administrators.
  • Become part of the hospital link programme, helping to facilitate supported discharge from the hospital by attending key ward meetings and visiting wards. Effectively communicate with team members, patients, and their family/whanau. Patient education and health promotion is pivotal to this role.

Ko ngāpūkengaōu | What you will bring

  • 5+ years’ experience as a Registered Nurse
  • A broad range of clinical skills across general practice, acute medicine, or urgent care
  • A high level of interpersonal, teamworking and communications skills
  • The ability to be adaptable and responsive, including the ability to think critically

All applicants must hold a current APC and NZNC Registration
